Resumen Introducción: La cirugía bariátrica se considera actualmente la opción de tratamiento más eficaz para la obesidad mórbida. El bypass gástrico laparoscópico en Y de Roux sigue siendo el estándar de oro en cirugía bariátrica. El advenimiento de la robótica quirúrgica ha llevado a una reducción de algunos de los desafíos más difíciles en la laparoscopia avanzada. Objetivo: Determinar la seguridad y eficacia del bypass gástrico en Y de Roux asistido por robot en comparación con el abordaje laparoscópico. Material y métodos: Se realizó un estudio retrospectivo que incluyo 50 pacientes con distintos grados de obesidad divididos en dos grupos de 25 cada uno. Se realizó una base de datos con las variables de acuerdo al tipo de abordaje quirúrgico (robótico y laparoscópico), registrando los días de estancia hospitalaria, sangrado, tiempo quirúrgico, complicaciones, reingresos hospitalarios, complicaciones y disminución del IMC. Resultados: En el grupo laparoscópico se observó un sangrado transoperatorio de 115.8+64 mililitros, en el grupo robótico solo fue de 59.6+45.8 mililitros (p<0.001). Un tiempo quirúrgico laparoscópico de 151.8+34.6 minutos, mientras que el grupo robótico fue de 216.4+50 minutos, los pacientes permanecieron hospitalizados 4.2+2.4 días en el grupo laparoscópico, los pacientes del grupo robótico 3.4+1 días, sin diferencia significativa (p=0.077). En ambos grupos no hubo reingresos hospitalarios. En el grupo laparoscópico la disminución de IMC fue de 8.9+2.5, mientras que para el grupo robótico fue de 13.7+2.3 con significancia estadística (p<0.001). Discusión: El bypass gástrico en Y de Roux asistido por robot es más seguro y eficaz en comparación con el abordaje laparoscópico. El abordaje robótico disminuye de forma significativa el sangrado transoperatorio, disminuye los días de estancia hospitalaria (sin diferencia significativa) y reduce de forma significativa la disminución del IMC, sin aumentar los reingresos hospitalarios a 30 días ni las complicaciones.


Abstract Introduction: Bariatric surgery is currently considered the most effective treatment option for morbid obesity. The laparoscopic Roux-en-Y gastric bypass remains the gold standard in bariatric surgery. The advent of surgical robotics has led to a reduction in some of the most difficult challenges in advanced laparoscopy. Objective: To determine the safety and efficacy of robot-assisted Roux-en-Y gastric bypass compared to the laparoscopic approach. Material and methods: A retrospective study was carried out that included 50 patients with different degrees of obesity divided into two groups of 25 patients each. A database was created with the variables according to the type of surgical approach (robotic and laparoscopic), recording the days of hospital stay, bleeding, surgical time, complications, hospital readmissions, complications, and BMI decrease. Results: In the laparoscopic group, intraoperative bleeding of 115.8+64 milliliters was observed, in the robotic group it was only 59.6+45.8 milliliters (p<0.001). A laparoscopic surgical time of 151.8+34.6 minutes, while the robotic group was of 216.4+50 minutes, the patients remained hospitalized 4.2+2.4 days in the laparoscopic group, the patients in the robotic group 3.4+1 days, with no significant difference (p=0.077). In both groups there were no hospital readmissions. In the laparoscopic group, the decrease in BMI was 8.9+2.5, while for the robotic group it was 13.7+2.3 with statistical significance (p<0.001). Discussion: Robot-assisted Roux-en-Y gastric bypass is more safe and effective compared to the laparoscopic approach. The robotic approach significantly reduces intraoperative bleeding, decreases the days of hospital stay (with no significant difference), and significantly reduces the decrease in BMI, without increasing 30-day hospital readmissions or complications.

ABSTRACT

La tecnología y la inteligencia artificial ganan cada vez más espacio en el diagnóstico, tratamiento y seguimiento del paciente: los wearables, las apps mHealth y los dispositivos basados en la tecnología robótica son un apoyo para el personal de salud de enfermería. A través del método hermenéutico se analizaron los resultados de una revisión bibliográfica de documentos seleccionados en Pubmed, Scielo y ELSEVIER, para describir el estado del arte de las wearables, apps y dispositivos tecnológicos que pueden ser empleados en el cuidado enfermero; lo que constituyó el objetivo de la presente investigación. Se procesaron diecisiete artículos y tres informes. Los resultados se muestran en tres categorías donde se identificaron: tres wereables, ocho aplicaciones móviles y tres robots que se emplean en enfermería. Se concluye que los wereables, las aplicaciones móviles y la robótica tienen hoy día una presencia trascendente en los espacios de cuidado y atención de salud y que acercan al personal de enfermería a sus pacientes. La tecnología no suple el cuidado de enfermería, pero si constituye una herramienta de apoyo en su quehacer.


Technology and artificial intelligence are gaining more and more space in patient diagnosis, treatment and monitoring; wearables, mHealth apps and devices based on robotic technology are a support for nursing health personnel. Through the hermeneutical method, the bibliographic review was carried out with the analysis and interpretation of the selected documents in Pubmed, Scielo and ELSEVIER: seventeen articles and three reports were processed. The results are shown in three categories identified as: three wearable, eight mobile applications and three robots used in nursing. The conclusion is that wearables, mobile applications and robotics today have a significant presence in healthcare spaces nursing staff closer to their patients. Technology does not replace nursing care but it is a support tool in their work.

ABSTRACT

Objective: The present study sought to retrospectively evaluate the efficacy of robotic gait training in children with cerebral palsy (CP) gross motor function classification system (GMFCS) levels II, III and IV. Method: The medical records from 69 patients with CP, who participated in the Lokomat® protocol, were analyzed using a retrospective approach. The results from the Gross Motor Function Measure (GMFM), 6-minute walk test (6MinWT), 10-meter walk test (10MWT) and Timed Up and Go (TUG) test were analyzed before and after the protocol was administered. Results: An improvement in GMFM was observed for all GMFCS levels. GMFCS level III patients showed a significant improvement in the 6MinWT (p= 0.01), and GMFCS level IV patients displayed a significant improvement in GMFM dimension B (p= 0.03). All tests showed a significant improvement when compared to their performance before the application of the protocol. Conclusion: The study suggests that all patients diagnosed with CP benefit from gait training, using the Lokomat® system, within their expected motor frame.


Objetivo: Verificar retrospectivamente a eficácia do treino de marcha com robótica (Lokomat®) em pacientes com Paralisia Cerebral (PC) níveis II, III e IV da Medida da Função Motora Grossa (GMFCS). Método: Análise retrospectiva descritiva do prontuário de 69 pacientes com PC que realizaram o protocolo da Lokomat®. Os resultados do teste de caminhada de 6 minutos (TC6M), teste de caminhada de 10 metros (TC10M), Timed Up and Go (TUG) e da Avaliação da Função Motora Grossa (GMFM) foram realizados e analisados pré e pós protocolo. Resultados: Foi observada uma melhora no GMFM para pacientes de todos os níveis do GMFCS. Pacientes GMFCS nível III apresentaram melhora significativa do TC6M (p= 0,01) e pacientes GMFCS nível IV apresentaram melhora significativa da dimensão B do GMFM (p= 0,03). Todos os testes mostraram melhoras significativas quando comparados aos resultados antes da aplicação do protocolo. Conclusão: O estudo sugere que todos os pacientes com diagnóstico de PC se beneficiaram do treino de marcha com uso da Lokomat® dentro de seu quadro motor esperado.

Introdução: A colecistectomia por incisão única assistida por robótica é técnica cirúrgica emergente para o tratamento da doença da vesícula biliar. Objetivo: Analisar os resultados clínicos e o custo efetividade dela, com foco no tempo de permanência hospitalar, tempo de operação, custo total e taxa de conversão entre robótica e outros procedimentos. Métodos: Revisão sistemática e metanálise foram realizadas de acordo com o Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Os bancos de dados PubMed, Embase e Cochrane foram pesquisados desde o início até março de 2023. Análise estatística foi feita usando o R versão 6.2.1. Metanálise de efeitos aleatórios com razão de risco, diferença média e intervalo de confiança de 95% foi estimada usando a variância inversa e o método de Mantel-Haenszel para resultados binários e o estimador DerSimonian-Laird para resultados contínuos. Resultados: Um total de 452 pacientes foram envolvidos, incluindo 4 estudos randomizados. Os desfechos escolhidos para metanálise foram: permanência hospitalar (MD −0.03 dias, CI 95% −0.12 a 0.18, p=0.708), tempo de operação (MD 12.93 min, CI 95% −21.40 a 47.25, p=0.460) e taxa de conversão (RR 0.90, CI 95% 0.44 a 1.83, p=0.771). Conclusão: Não houve diferença estatisticamente significativa em relação à duração da permanência hospitalar, tempo de operação e taxa de conversão entre a colecistectomia robótica por incisão única e outras técnicas cirúrgicas para a doença da vesícula biliar.


Introduction: Robotic-assisted single-incision cholecystectomy is an emerging surgical technique for the treatment of gallbladder disease. Objective: To analyze the clinical results and its cost effectiveness, focusing on length of hospital stay, operating time, total cost and conversion rate between robotics and other procedures. Methods: Systematic review and meta-analysis were performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. PubMed, Embase, and Cochrane databases were searched from inception through March 2023. Statistical analysis was performed using R version 6.2.1. Random effects meta-analysis with hazard ratio, mean difference and 95% confidence interval was estimated using inverse variance and the Mantel-Haenszel method for binary outcomes and the DerSimonian-Laird estimator for continuous results. Results: A total of 452 patients were enrolled, including 4 randomized trials. The outcomes chosen for meta-analysis were: hospital stay (MD −0.03 days, CI 95% −0.12 to 0.18, p=0.708), operating time (MD 12.93 min, CI 95% −21.40 to 47.25, p=0.460) and of conversion (RR 0.90, CI 95% 0.44 to 1.83, p=0.771). Conclusion: There was no statistically significant difference regarding length of hospital stay, operating time and conversion rate between single-incision robotic cholecystectomy and other surgical techniques for gallbladder disease.

ABSTRACT

Introducción. La recuperación de la marcha es uno de los principales objetivos en rehabilitación después de un ACV. Basados en los principios de aprendizaje motor, se han desarrollado nuevas estrategias en neurorrehabilitación basadas en la práctica repetitiva, orientada a la tarea y la retroalimentación. Esto último ha demostrado ser una de las variables clave para el entrenamiento, por su fácil obtención y manipulación. Sin embargo, aún no existen estudios concluyentes que permitan identificar el efecto real de esta variable y su influencia en la recuperación y el desempeño funcional de la marcha. Objetivo. Determinar el efecto de la retroalimentación visual sobre la velocidad de la marcha después de un accidente cerebrovascular en adultos con estadios subagudos y crónicos. Metodología. Diseño de caso único de línea de base múltiple, aleatorio no concurrente de cuatro participantes. Se evaluó la velocidad de la marcha determinando las diferencias en el nivel, la tendencia, la estabilidad de los datos y la no superposición de datos mediante el análisis visual basado en la documentación técnica para diseños de caso único de la What Works Clearinghouse. Resultados. Cuatro participantes con rango de edad de 19 a 73 años fueron incluidos en el estudio. El cambio en el nivel para todos los participantes demostró un incremento en los valores de la velocidad de la marcha después de la introducción de la intervención (media: 0.76 m/s). El análisis visual de la tendencia estimó aceleración para la línea de intervención para tres participantes. Los datos en la fase de base e intervención cumplieron el criterio de estabilidad medido con el método de banda de dos desviaciones estándar (media: 0.05 m/s); los patrones de cambio demostraron efecto inmediato con mejoría gradual durante la intervención para los participantes 1, 3 y 4. El porcentaje de no superposición de datos mostró efectividad de la intervención para tres de los participantes (PND >91.67%). Conclusiones. Los hallazgos presentados en este estudio representan un aporte científico que respalda la pertinencia del uso y aplicación de los principios de aprendizaje motor para el desarrollo de nuevas estrategias en rehabilitación motora. Sin embargo, este estudio constituye un primer paso para realizar estudios más robustos que incluyan replicación de las fases en el estudio y la evaluación del seguimiento para determinar la permanencia de los efectos a largo plazo.


Introduction. Gait recovery is one of the main goals in post-stroke rehabilitation. Based on the principles of motor learning, new strategies have been developed in neurorehabilitation based on repetitive, task-oriented practice, and feedback. The latter has proven to be one of the most critical variables for training, because it is easy to obtain and manipulate. However, there are still no conclusive studies to identify the real effect of this variable and its influence on recovery and functional gait performance. Objective. To determine the effect of visual feedback on gait speed after stroke in adults with subacute and chronic stages.Methodology. Single-case, multiple baseline, non-concurrent randomized, and four-participant design. Gait velocity was assessed by determining differences in level, trend, data stability, and nonoverlapping data using visual analysis based on technical documentation for single-case designs from the What Works Clearinghouse.Results. Four participants ranging in age from 19 to 73 years were included in the study. The change in level for all participants demonstrated an increase in gait velocity values after the introduction of the intervention (mean: 0.76 m/s). Visual trend analysis estimated acceleration for the intervention line for three participants. The data in the baseline and intervention phase met the stability criterion measured with the two standard deviation band method (mean: 0.05 m/s); patterns of change demonstrated immediate effect with gradual improvement during the intervention for participants 1, 3, and 4. The percentage of nonoverlapping data showed effective-ness of the intervention for three of the participants (PND >91.67%).Conclusions. The findings presented in this study represent a scientific contribution that supports the relevance of the use and application of motor learning principles for the development of new strategies in motor rehabilitation. However, this study constitutes a first step towards more robust studies that include replication of the phases in the study and follow-up evaluation to determine the permanence of long-term effects.

INTRODUCCIÓN. La cirugía robótica se realiza gracias al sistema telemanipulado, alcanzando tareas repetitivas, precisas y mínimamente invasivas. En Ecuador inició en el año 2016, incluyendo este procedimiento a pediatría en el 2021. OBJETIVO. Describir las características clínicas y resultados quirúrgicos de pacientes pediátricos intervenidos por cirugía robótica. MATERIALES Y MÉTODOS. Estudio transversal analítico. Población de 278 y muestra de 40 datos de historias clínicas electrónicas de pacientes pediátricos intervenidos por cirugía robótica de agosto del 2021 a marzo del 2022 en el Hospital Carlos Andrade Marín. Criterios de inclusión: pacientes pediátricos intervenidos por cirugía robótica en el Hospital de Especialidades Carlos Andrade Marín. Criterios de exclusión: pacientes adultos, haber sido intervenidos quirúrgicamente por técnicas abiertas o laparoscópica. La información se obtuvo del sistema AS400; se creó una base de datos anonimizando a los pacientes. Se aplicó un análisis univariado y para determinar significancia estadística se utilizó la prueba de Chi-2. La información recolectada fue analizada en el programa estadístico International Business Machines Statistical Package for the Social Sciences. RESULTADOS: De los pacientes del estudio: 45,00% (18; 40) era adolescente. El 52,50% (21; 40) no presentó comorbilidades. 50,00% (20; 40) de las intervenciones fueron colecistectomía. 55% (22; 40) fué por diagnóstico prequirúrgico y posquirúrgico gastrointestinal y hepático. El 95,00% (38; 40) no presentó complicaciones. El 100% (40; 40) presentó sangrado G1. El 97,50% (39; 40) no presentó eventos, y el 72,5% (29; 40) no presentó dolor. El promedio de peso de los pacientes fue 41 kilogramos, con una media de 3 días de estancia hospitalaria, un promedio de 65 minutos de cirugía total y 15 minutos de docking, los cuales son estadísticamente significativos con una p= 0,001. CONCLUSIONES. Se registró las características clínicas y resultados quirúrgicos de pacientes pediátricos intervenidos por cirugía robótica. La cirugía robótica se aplicó con seguridad en pacientes pediátricos del Hospital Carlos Andrade Marín.


INTRODUCTION. Robotic surgery is performed thanks to the telemanipulated system, achieving repetitive, precise and minimally invasive tasks. In Ecuador it started in the Hospital de Especialidades Carlos Andrade Marín in 2016, including this procedure to pediatrics in 2021. OBJECTIVE. To describe the clinical characteristics and surgical outcomes of pediatric patients operated by robotic surgery. MATERIALS and METHODS. Analytical cross-sectional study. Population of 278 and sample of 40 data from electronic medical records of pediatric patients operated by robotic surgery from August 2021 to March 2022 at the Carlos Andrade Marin Hospital. Inclusion criteria: pediatric patients who underwent robotic surgery at the Hospital de Especialidades Carlos Andrade Marín. Exclusion criteria: adult patients, having undergone open or laparoscopic surgery. The information was obtained from the AS400 system; a database was created by anonymizing the patients. Univariate analysis was applied and the Chi-2 test was used to determine statistical significance. The information collected was analyzed in the statistical program International Business Machines Statistical Package for the Social Sciences. RESULTS: Of the patients in the study: 45,00% (18; 40) were adolescents. 52,50% (21; 40) had no comorbidities. 50,00% (20; 40) of the interventions were cholecystectomy. 55% (22; 40) were for preoperative and postoperative gastrointestinal and hepatic diagnosis. 95,00% (38; 40) had no complications. 100% (40; 40) presented G1 bleeding. 97.50% (39; 40) did not present events, and 72,5% (29; 40) did not present pain. The average weight of the patients was 41 kilograms, with a mean hospital stay of 3 days, an average of 65 minutes of total surgery and 15 minutes of docking, which are statistically significant with a p= 0,001. CONCLUSIONS. The clinical characteristics and surgical results of pediatric patients who underwent robotic surgery were recorded. Robotic surgery was safely applied in pediatric patients at the Carlos Andrade Marin Hospital.

Resumen A pesar de la evidencia científica de la seguridad, la eficacia de la cirugía robótica, existen pocas publicaciones en la reparación de la vía biliar. El principal objetivo de este estudio fue evaluar los resultados de la hepaticoyeyunostomía en Y de Roux asistida por robot (HYR) y la hepaticoyeyunostomía en Y de Roux abierta (HYA) en el HCM en el periodo de enero de 2012 a enero de 2019. Este es un estudio retrospectivo se recolectaron los datos sociodemográficos, las principales comorbilidades y se determinó la principal indicación quirúrgica de la hepaticoyeyunostomía; se agruparon según la indicación quirúrgica para comparar el sangrado transoperatorio, tiempo quirúrgico, los días de estancia hospitalaria y morbilidad entre la HYR vs HYA. Se registraron 78 hepaticoyeyunostomías (31 fueron HYR y 47 HYA), la edad promedio fue 55.2 ± 17.7 años predominio del género femenino, la principal comorbilidad fue la hipertensión arterial. La principal indicación quirúrgica de hepaticoyeyunostomía fue la disrupción de vía la biliar, en estos pacientes se encontró menor tiempo quirúrgico, días de hospitalización, y morbilidad en los pacientes sometidos a HYR que los pacientes tratados mediante HYA.


Abstract Despite the scientific evidence of safety, the efficacy of robotic surgery, there are few publications on bile duct repair. The main objective of this study was to evaluate the results of Robotic-assisted Hepaticojejunostomy (RHJ) versus open Hepaticoyejunostomy (OHJ) in the HCM during the period from January 2012 to January 2019. This is a retrospective study, database of patients was collected: sociodemographic data, the main comorbidities, and the main surgical indication of hepaticojejunostomy were determined. They were grouped according to the surgical indication to compare: transoperative bleeding, surgical time, hospital length of stay and morbidity RHJ vs. OHJ. Seventy-eight hepaticojejunostomies were registered (31 RHJ and 47 OHJ), the average age was 55.2 ± 17.7 years, predominantly female, the main comorbidity was arterial hypertension. The main surgical indication of hepaticoyeynostomy was bile duct disruption. Shorter surgical time, days of hospitalization, and morbidity were found in patients undergoing RHJ than patients treated with OHJ.

Introducción: La cirugía mínimamente invasiva del páncreas se enmarca dentro de la actual historia de la cirugía con gran impacto y desarrollo, especialmente en el campo de las pancreatectomías distales. Objetivo: Actualizar los aspectos generales y actuales de la cirugía mínimamente invasiva en las afecciones pancreáticas. Métodos: Se realizó una revisión sistemática a partir de la consulta de artículos científicos relacionados con el tema, indexados en las bases de datos Pubmed, Ebsco y SciELO. La información se analizó y se seleccionaron artículos publicados hasta 2020, relacionados con las indicaciones de cirugía laparoscópica y la robótica en las afecciones pancreáticas, la variabilidad de procederes quirúrgicos laparoscópicos y la morbimortalidad. Desarrollo: La cirugía mínimamente invasiva del páncreas ha alcanzado considerables niveles de complejidad y seguridad (desde la laparoscopia diagnóstica hasta las grandes resecciones pancreáticas). Se identifican las indicaciones de cirugía laparoscópica y la robótica en las afecciones pancreáticas, la variabilidad de procederes quirúrgicos laparoscópicos y se exponen los procederes realizados en el Centro Nacional de Cirugía de Mínimo Acceso durante estos últimos 3 años por el grupo dedicado a esta entidad. Conclusiones: La cirugía mínimamente invasiva y la robótica en afecciones pancreáticas son capaces de ofrecer resultados satisfactorios, siempre que sean realizadas por cirujanos con experiencia en cirugía hepato-biliopancreática y cirugía laparoscópica. Garantiza un riesgo de intervención mucho menor y una óptima recuperación en el menor tiempo posible con resultados similares en cuanto a morbimortalidad con la cirugía convencional(AU)


Introduction: Minimally invasive pancreatic surgery is framed within the current history of surgery with great impact and development, especially in the field of distal pancreatectomies. Objective: To update the general and current aspects of minimally invasive surgery in pancreatic disorders. Methods: A systematic review was carried out based on the consultation of scientific articles about the subject, indexed in the Pubmed, Ebsco and SciELO databases. The information was analyzed and articles published up to 2020 were selected, related to the indications for laparoscopic and robotic surgery in pancreatic conditions, the variability of laparoscopic surgical procedures, as well as morbidity and mortality. Development: Minimally invasive pancreatic surgery has reached considerable levels of complexity and safety (from diagnostic laparoscopy to large pancreatic resections). The indications for laparoscopic and robotic surgery in pancreatic disorders, together with the variability of laparoscopic surgical procedures, are identified, and the procedures performed at the National Center for Minimally Access Surgery during the last three years by the group dedicated to this entity are presented. Conclusions: Minimally invasive surgery and robotic surgery in pancreatic affections can offer satisfactory outcomes, as long as they are performed by surgeons with experience in hepatobiliopancreatic surgery and laparoscopic surgery. It guarantees a much lower risk of intervention and optimal recovery in the shortest possible time, with similar outcomes in terms of morbidity and mortality to conventional surgery(AU)

ABSTRACT

Abstract Objective To summarize the available evidence of TAP Block in efficacy in laparoscopic or robotic hysterectomy. Data Sources We searched databases and gray literature for randomized controlled trials in which transversus abdominis plane (TAP) block was compared with placebo or with no treatment in patients who underwent laparoscopic or robot-assisted hysterectomy. Method of Study Selection Two researchers independently evaluated the eligibility of the selected articles. Tabulation, Integration, and Results Seven studies were selected, involving 518 patients. Early postoperative pain showed a difference in the mean mean difference (MD): - 1.17 (95% confidence interval [CI]: - 1.87-0.46) in pain scale scores (I2=68%), which was statistically significant in favor of using TAP block, but without clinical relevance; late postoperative pain: DM 0.001 (95%CI: - 0.43-0.44; I2=69%); opioid requirement: DM 0.36 (95%CI: - 0.94-1.68; I2=80%); and incidence of nausea and vomiting with a difference of 95%CI=- 0.11 (- 0.215-0.006) in favor of TAP. Conclusion With moderate strength of evidence, due to the high heterogeneity and imbalance in baseline characteristics among studies, the results indicate that TAP block should not be considered as a clinically relevant analgesic technique to improve postoperative pain in laparoscopic or robotic hysterectomy, despite statistical significance in early postoperative pain scale scores. Resumo Objetivo Resumir as evidências disponíveis sobre a eficácia do bloqueio TAP em histerectomia laparoscópica ou robótica. Fontes de Dados Pesquisamos bancos de dados e literatura cinza por ensaios clínicos randomizados nos quais o bloqueio do plano transverso do abdome (TAP na sigla em inglês) foi comparado com placebo ou com nenhum tratamento em pacientes que foram submetidos a histerectomia laparoscópica ou assistida por robô. Métodos de Seleção de Estudos Dois pesquisadores avaliaram independentemente a elegibilidade dos artigos selecionados. Tabulação, Integração e Resultados Sete estudos foram selecionados envolvendo 518 pacientes. A dor pós-operatória precoce apresentou diferença nasmédias (DM) de: -1 17 (intervalo de confiança [IC] de 95%: - 1 87-0 46) nos escores da escala de dor (I2=68%) o que foi estatisticamente significativo a favor do uso do bloqueio TAP mas sem relevância clínica; dor pós-operatória tardia: DM 0001 (IC95%: - 043-044; I2=69%); necessidade de opioides: DM0 36 (95%CI: - 0 94-168; I2=80%); e incidência de náuseas e vômitos com diferença de 95% CI=- 011 (- 0215-0006) a favor do TAP. Conclusão Com moderada força de evidência devido à alta heterogeneidade e ao desequilíbrio nas características basais entre os estudos os resultados indicam que o bloqueio do TAP não deve ser considerado como uma técnica analgésica clinicamente relevante para melhorar a dor pós-operatória em histerectomia laparoscópica ou robótica apesar da significância estatística nas pontuações da escala de dor pósoperatória inicial. ABSTRACT Objectives: to build and validate educational technology, of the video type, aimed at teaching about the perioperative period of robotic surgery. Methods: methodological development study, following the integrative review phases: pre-production, production, post-production, and validation. The video was evaluated for content and appearance by 14 judges. Content validity (CVI) and appearance (AVI) and agreement (CI) indices were calculated, and the exact binomial test was applied, considering p>0.05 and a proportion of 0.80 of agreement. Results: the video included 13 themes, with a duration of seven minutes and 33 seconds. The items of the three dimensions evaluated presented excellent CVI, ranging between 0.86 and 1.00 (p>0.05), with a total CVI of 0.95, VAT of 0.94 and CI equal to 61.5%. RESUMEN Objetivos: construir y validar tecnología educacional, del tipo vídeo, dirigida para la enseñanza acerca del perioperatorio de cirugía robótica. Métodos: estudio de desarrollo metodológico, siguiendo las fases de revisión integrativa: preproducción, producción, postproducción y validación. El vídeo fue evaluado cuanto al contenido y apariencia por 14 jueces. Calculados índices de validez de contenido (IVC) y apariencia (IVA), concordancia (IC), y aplicado test exacto de binomial, considerando p>0,05 y proporción de 0,80 de concordancia. Resultados: el vídeo contempló 13 temáticas, con tiempo de duración de siete minutos y 33 segundos. Los ítems de las tres dimensiones evaluadas presentaron IVCs excelentes, variando entre 0,86 y 1,00 (p>0,05), con IVC total de 0,95, IVA de 0,94 e IC igual a 61,5%. Conclusiones: construido herramienta tecnológica, del tipo vídeo educativo, para la enseñanza sobre el perioperatorio de cirugía robótica. ABSTRACT Introduction: the botulinum toxin A (BTA) has been used to achieve a chemical component separation, and it has been used with favorable outcomes for the repair of complex ventral hernia (CVH) with and without loss of domain (LD). The aim of this study is to describe our early experience with the chemical component separation in the United Sates. Methods: a retrospective observational study of all patients who underwent ventral hernia repair for CVH with or without LD between July 2018 and June 2020. Preoperative BTA was injected in all patients via sonographic guidance bilaterally, between the lateral muscles to achieve chemical denervation before the operation. Patient demographics, anatomical location of the hernia, perioperative data and postoperative data are described. Results: 36 patients underwent this technique before their hernia repair between July 2018 to June 2020. Median age was 62 years (range 30-87). Median preoperative defect size was 12cm (range 6-25) and median intraoperative defect size was 13cm (range 5-27). Median preoperative hernia sac volume (HSV) was 1338cc (128-14040), median preoperative abdominal cavity volume (ACV) was 8784cc (5197-18289) and median volume ration (HSV/ACV) was 14%. The median OR time for BTA administration was 45 minutes (range 28-495). Seroma was the most common postoperative complication in 8 of the patients (22%). Median follow up was 43 days (range 0-580). Conclusion: preoperative chemical component separation with BTA is a safe and effective adjunct to hernia repair in CVH repairs where a challenging midline fascial approximation is anticipated.


RESUMO Introdução: a toxina botulínica A (TBA) tem sido usada para alcançar a separação química dos componentes, com resultados favoráveis para o reparo de hérnias ventrais complexas (HVC) com ou sem perda de domínio (PD). O objetivo deste estudo é descrever nossa experiência inicial com a separação química dos componentes nos Estados Unidos. Métodos: estudo retrospectivo observacional com todos os pacientes submetidos a reparo de hérnia ventral complexa com ou sem PD entre julho de 2018 e junho de 2020. A TBA pré-operatória foi injetada em todos os pacientes, guiada por ultrassonografia, bilateralmente entre os músculos laterais para alcançar a denervação química antes da operação. Dados demográficos dos pacientes, local da hérnia, dados peri-operatórios e pós-operatórios são descritos. Resultados: 36 pacientes foram submetidos a esta técnica antes do reparo da hérnia. A idade mediana foi 62 anos (30-87). O tamanho mediano pré-operatório do defeito herniário foi 12cm (6-25) e o defeito mediano intra-operatorio foi 13cm (5-27). O volume mediano do saco herniário pré-operatório foi 1.338mL (128-14.040), o volume mediano da cavidade abdominal pré-operatório foi 8.784 (5.197-18.289) mL e a razão dos volumes foi 14%. A duração mediana da operação para aplicação da TBA foi 45 minutos (28-495). O seroma foi a complicação mais comum no pós-operatório, em oito pacientes (22%). O seguimento pós-operatório mediano foi de 43 dias (0-580). Conclusão: a separação pré-operatória de componentes com TBA é tratamento adjuvante seguro e eficaz em reparos de HVC em que aproximação da fáscia é antecipadamente desafiadora.

ABSTRACT Objective: to describe the use of the robotic platform in inguinal hernia recurrence after a previous laparoscopic repair. Methods: patients with recurrent inguinal hernias following a laparoscopic repair who have undergone robotic transabdominal preperitoneal between December 2015 through September 2020 were identified in a prospectively maintained database. Outcomes of interest included demographics, hernia characteristics, operative details and rates of 30-day surgical site occurrence, surgical site occurrences requiring procedural interventions, surgical site infection and hernia recurrence were abstracted. Results: nineteen patients (95% male, mean age 55 years, mean body mass index 28) had 27 hernias repaired (N=8 bilateral). Average operative time was 168.9 ± 49.3min (range 90-240). There were two intraoperative complications all of them were bleeding from the inferior epigastric vessel injuries. Three SSOs occurred (N=2 seromas and N=1 hematoma. After a median 35.7 months follow-up (IQR 13-49), no recurrence has been diagnosed. One patient developed chronic postoperative inguinal pain. Conclusions: on a small number of selected patients and experienced hands, we found that the use of the robotic platform for repair of recurrent hernias after prior laparoscopic repair appears to be feasible, safe and effective despite being technically demanding. Further studies in larger cohorts are necessary to determine if this technique provides any benefits in recurrent inguinal hernia scenario.


RESUMO Objetivo: descrevemos nossa experiência com uso da plataforma robótica no tratamento das recidivas operadas previamente por laparoscopia, mantendo assim uma proposta minimamente invasiva a esses pacientes, apesar de haver uma predileção pela via anterior e aberta nestes casos. Métodos: foram incluídos pacientes submetidos a hernioplastia inguinal robótica transabdominal pré-peritoneal como tratamento de recidiva e que foram operados previamente por laparoscopia, entre dezembro de 2015 e setembro de 2020 e mantidos em uma base de dados ambulatorial prospectiva. Variáveis de interesse incluíram dados demográficos, características herniárias, detalhes operatórios, ocorrências do sítio cirúrgico em 30 dias (com ou sem necessidade de intervenção), infeção do sítio cirúrgico, tempo de seguimento e taxa de recidiva. Resultados: dezenove pacientes (95% masculino, média de idade de 55 anos, média de índice de massa corporal 28kg/m2) e 27 hérnias operadas (N=8 bilaterais). Média de tempo cirúrgico 168.9±49.3 min (variando 90-240). N=2 complicações intraoperatórias por lesão de vasos epigástricos inferiores. N=2 seromas e N=1 hematoma foram identificados no pós-operatório; N=1 paciente apresentou dor crônica pós operatória. Após um tempo de seguimento médio de 35.7 meses (intervalo entre quartis 13-49), nenhuma recidiva foi diagnosticada. Conclusões: o uso da plataforma robótica parece ser seguro e efetivo no tratamento das recidivas operadas previamente laparoscopia, nesse pequeno grupo de pacientes selecionados, apesar de requerer expertise em cirurgia robótica. Outros estudos com maiores casuísticas são necessários para estabelecer o papel desta técnica no cenário das hérnias inguinais recidivadas.

Abstract Introduction Prostatectomy is the standard treatment for patients with clinically localized prostate cancer. Currently, robot-assisted radical prostatectomy (RARP) is widely used for its advantages, as it provides better visualization, precision, and reduced tissue manipulation. However, RARP requires a multidisciplinary approach in which anesthesia and analgesia management are especially important. Objective This study aims to describe our experience delivering anesthesia for the first cases of patients undergoing RARP in a teaching hospital in Bogotá, Colombia. Methodology An observational study was conducted. We included all patients undergoing RARP from September 2015 to December 2019 at Fundación Santa Fe de Bogotá. All patients with incomplete data were excluded. Patient demographics were recorded, and significant perioperative events were reviewed. Results A total of 301 patients were included. At our institution, the mean age for patients undergoing RARP was 61.4 ± 6.7 years. The mean operative time was 205 ± 43 min and mean blood loss was 300 [200400] mL. Only 6 (2%) patients required transfusion. Age and BMI were not associated with clinical outcomes. Conclusions An adequate perioperative approach in RARP is important to minimize complications, which in this study and in this institution were infrequent.


Resumen Introducción La prostatectomía es el tratamiento estándar para pacientes con cáncer de próstata localizado. Actualmente, la prostatectomía radical asistida por robot es ampliamente utilizada por sus ventajas en visualización, precisión y manipulación de los tejidos. Sin embargo, este abordaje requiere un manejo multidisciplinario, pues el enfoque analgésico y anestésico es fundamental para optimizar los desenlaces. Objetivo Describir los primeros casos de prostatectomía radical asistida por robot realizadas en un hospital universitario de cuarto nivel en Bogotá, Colombia. Metodología Estudio observacional en el cual se incluyeron todos los pacientes sometidos a prostatectomía radical asistida por robot (PRAR) en el hospital Fundación Santa Fe de Bogotá entre septiembre de 2015 y diciembre de 2019. Se excluyeron los pacientes con historia clínica incompleta. Se registraron los datos demográficos y se revisaron los eventos perioperatorios importantes. Resultados Se analizaron 301 pacientes. La edad media de pacientes sometidos a PRAR fue 61,4 ± 6,7 años. El tiempo quirúrgico promedio fue 205 ± 43 minutos y la pérdida sanguínea media fue 300 [200-400] mL. Solo 6 pacientes (2 %) requirieron transfusión. La edad y el IMC no mostraron una asociación relevante con los desenlaces clínicos. Conclusiones El adecuado abordaje perioperatorio en PRAR es importante para minimizar las complicaciones, las cuales en este estudio y en esta institución fueron infrecuentes.

ABSTRACT

Resumen La enfermería no es ajena al cambio revolucionario que supone la introducción de la inteligencia artificial en el cuidado de la salud. A principios de 2021 se publicaron las conclusiones del think-tank internacional sobre la inteligencia artificial y la enfermería, en las que se reconoce la relevancia del uso de dichas tecnologías para aumentar y extender las posibilidades de esta disciplina, entre ellas, el cuidado. Una valoración ponderada acerca del acierto de estas conclusiones exige, necesariamente, una reflexión epistemológica sobre el cuidado. En el presente artículo reflexionaremos sobre la incidencia de la inteligencia artificial en el cuidado de enfermería desde la perspectiva de la ética del cuidado y a la luz de los principales aportes de Samaritanus Bonus.


Abstract Nursing is no stranger to the revolutionary change brought about by the introduction of artificial intelligence in health care. At the beginning of 2021, the international think-tank on artificial intelligence and nursing published its conclusions, recognizing the relevance of these technologies to increase and extend the capacities of this discipline, including care. A weighted assessment of the correctness of these conclusions necessarily requires an epistemological reflection on care. This article considers the impact of artificial intelligence on nursing care from the ethics of care in light of the main contributions of the Samaritanus Bonus.


Resumo A enfermagem não está alheia à mudança revolucionária que a introdução da inteligência artificial no cuidado da saúde implica. No início de 2021, foram publicadas as conclusões do think-tank internacional sobre a inteligência artificial e a enfermagem, nas quais é reconhecida a relevância do uso dessas tecnologias para aumentar e ampliar as capacidades dessa disciplina, entre elas, o cuidado. Uma avaliação ponderada sobre a assertividade dessas conclusões exige, necessariamente, uma reflexão epistemológica acerca do cuidado. Neste artigo, refletimos sobre a incidência da inteligência artificial no cuidado de enfermagem sob a perspectiva da ética do cuidado e à luz das principais contribuições do Samaritanus Bonus.

ABSTRACT BACKGROUND: Robotic surgery has gained growing acceptance in recent years, expanding to liver resection. OBJECTIVE: The aim of this paper is to report the experience with our first fifty robotic liver resections. METHODS: This was a single-cohort, retrospective study. From May 2018 to December 2020, 50 consecutive patients underwent robotic liver resection in a single center. All patients with indication for minimally invasive liver resection underwent robotic hepatectomy. The indication for the use of minimally invasive technique followed practical guidelines based on the second international laparoscopic liver consensus conference. RESULTS: The proportion of robotic liver resection was 58.8% of all liver resections. Thirty women and 20 men with median age of 61 years underwent robotic liver resection. Forty-two patients were operated on for malignant diseases. Major liver resection was performed in 16 (32%) patients. Intrahepatic Glissonian approach was used in 28 patients for anatomical resection. In sixteen patients, the robotic liver resection was a redo hepatectomy. In 10 patients, previous liver resection was an open resection and in six it was minimally invasive resection. Simultaneous colon resection was done in three patients. One patient was converted to open resection. Two patients received blood transfusion. Four (8%) patients presented postoperative complications. No 90-day mortality was observed. CONCLUSION: The use of the robot for liver surgery allowed to perform increasingly difficult procedures with similar outcomes of less difficult liver resections.


RESUMO CONTEXTO: A cirurgia robótica tem tido aceitação crescente nos últimos anos, expandindo-se para a ressecção hepática. OBJETIVO: Relatar a experiência com as primeiras cinquenta ressecções hepáticas robóticas. MÉTODOS: Trata-se de análise retrospectiva de dados coletados prospectivamente. De maio de 2018 a dezembro de 2020, 50 pacientes consecutivos foram submetidos à ressecção hepática robótica em um único centro. Todos os pacientes com indicação de ressecção hepática minimamente invasiva foram submetidos à hepatectomia robótica. A indicação de técnica minimamente invasiva seguiu as diretrizes práticas baseadas na segunda conferência internacional de consenso laparoscópico hepático. RESULTADOS: A proporção de ressecções hepáticas robóticas foi de 58,8% de todas as ressecções hepáticas. Trinta mulheres e 20 homens com idade mediana de 61 anos foram submetidos à ressecção hepática robótica. Quarenta e dois pacientes foram operados por doenças malignas. Ressecção hepática maior foi realizada em 16 (32%) pacientes. A abordagem Glissoniana intra-hepática foi usada em 28 pacientes para ressecção anatômica. Em 16 pacientes, a ressecção hepática robótica foi uma re-hepatectomia. Em 10, a hepatectomia prévia foi aberta e em seis foi por via minimamente invasiva. Ressecção simultânea do cólon foi feita em três pacientes. Um paciente foi convertido para ressecção aberta. Dois pacientes receberam transfusão sanguínea. Quatro (8%) pacientes apresentaram complicações pós-operatórias. Mortalidade em 90 dias foi nula. CONCLUSÃO: O uso do robô permitiu realizar procedimentos progressivamente mais complexos com resultados semelhantes às hepatectomias menos complexas.

ABSTRACT

RESUMO O objetivo deste estudo foi propor um protocolo de ensaio clínico randomizado para avaliar o efeito da reabilitação robótica sobre a funcionalidade de pacientes com acidente vascular cerebral (AVC) subagudo. Trata-se de um protocolo de um ensaio clínico randomizado que será desenvolvido no hospital e centro de reabilitação do Pavilhão Pereira Filho da Irmandade da Santa Casa de Misericórdia de Porto Alegre (ISCMPA). Quarenta pacientes com AVC, de ambos os sexos, com idades entre 18 e 85 anos e que apresentem hemiparesia ou fraqueza muscular (Medical Research Council - MRC <48 pontos) serão divididos aleatoriamente em grupo controle ou grupo de intervenção. O grupo de intervenção será aquele que realizará reabilitação robótica utilizando equipamento Erigo®, além da fisioterapia convencional, e o grupo controle receberá fisioterapia convencional por meio de exercícios com movimentos semelhantes aos realizados no robô. As intervenções ocorrerão todos os dias na fase hospitalar e, após a alta, três vezes por semana, totalizando aproximadamente 18 sessões. A funcionalidade será considerada o desfecho primário do estudo e será avaliada por meio da escala de Fugl-Meyer. Consideramos como desfechos secundários a força muscular (MRC e teste de repetição máxima), espasticidade (escala de Ashworth modificada), arquitetura do músculo quadríceps e ecogenicidade (ultrassom), mobilidade (teste timed up and go), grau de incapacidade e dependência (escala de Rankin e de medida de independência funcional), qualidade de vida (questionário EQ-5D), repercussões cardiorrespiratórias (monitoramento de sinais vitais), tempo de internação (em dias) e mortalidade (número de óbitos). Os grupos serão avaliados antes das intervenções, após a décima sessão e ao final de seis semanas de tratamento ou 18 sessões.


ABSTRACT The aim of this study was to propose a randomized clinical trial protocol to evaluate the effect of robotic rehabilitation on the functionality of patients with subacute stroke. This is a protocol of a randomized clinical trial that will be developed at hospital and rehabilitation center of Pavilhão Pereira Filho of ISCMPA. We will recruit 40 patients with stroke, all genders, aged between 18 and 85 years and showing hemiparesis or muscle weakness (Medical Research Council- MRC<48 points), who will be randomized to a control group or intervention group. The intervention group-besides conventional physical therapy-will perform robotic rehabilitation using Erigo® equipment. The control group, in its turn, will receive conventional physical therapy executing exercises with similar movements to those performed on the robot. Interventions will occur every day during hospital phase and three times/week after discharge, totaling approximately 18 sessions. Functioning will be considered the primary outcome of the study and will be assessed using the Fugl-Meyer scale. As secondary outcomes, we considered: muscle strength (MRC and maximum repetition test); spasticity (modified Ashworth scale); quadriceps muscle architecture and echogenicity (ultrasound); mobility (timed up go test); degree of disability and dependence (Rankin scale and Functional Independence Measure); quality of life (EQ-5D questionnaire); cardiorespiratory repercussions (monitoring vital signs); length of hospital stay (in days); and mortality (number of deaths). The groups will be evaluated before the interventions, after the 10th session, and at the end of six weeks of treatment or 18 sessions.


RESUMEN El objetivo de este estudio fue proponer un protocolo de ensayo clínico aleatorizado para evaluar el efecto de la rehabilitación robótica en la funcionalidad de pacientes con accidente cerebrovascular subagudo. Se trata de un protocolo de ensayo clínico aleatorizado que será desarrollado en el hospital y centro de rehabilitación del Pavilhão Pereira Filho da Irmandade de la Santa Casa de Misericórdia de Porto Alegre (ISCMPA). Se asignarán aleatoriamente a cuarenta pacientes con ACV, de ambos sexos, con edades entre 18 y 85 años, que presenten hemiparesia o debilidad muscular (Medical Research Council - MRC <48 puntos) a un grupo de control o grupo de intervención. El grupo de intervención realizará rehabilitación robótica utilizando la herramienta Erigo® y fisioterapia convencional, mientras que el grupo de control recibirá fisioterapia convencional mediante ejercicios con movimientos similares a los realizados en el robot. Las intervenciones se realizarán todos los días durante la fase hospitalaria y, tras el alta, tres veces por semana, totalizando aproximadamente 18 sesiones. La funcionalidad se considerará el resultado primario del estudio y se evaluará mediante la escala de Fugl-Meyer. Se consideraron como resultados secundarios la fuerza muscular (MRC y test de máxima repetición), la espasticidad (escala de Ashworth modificada), la arquitectura del músculo cuádriceps y ecogenicidad (ultrasonido), la movilidad (test timed up and go), el grado de discapacidad y dependencia (escala de Rankin y medida de independencia funcional), la calidad de vida (cuestionario EQ-5D), la repercusión cardiorrespiratoria (seguimiento de constantes vitales), la estancia hospitalaria (en días) y la mortalidad (número de defunciones). Los grupos serán evaluados antes de las intervenciones, después de la décima sesión y al final de las seis semanas de tratamiento, es decir, 18 sesiones.

ABSTRACT

Resumen El síndrome de apnea e hipoapnea obstructiva del sueño (SAHOS) es una condición frecuente, que se asocia a importantes consecuencias negativas. El tratamiento inicial suele ser médico, pero en pacientes seleccionados la cirugía tiene excelentes resultados. En los últimos años se ha desarrollado la cirugía robótica transoral para tratar a pacientes con obstrucción anatómicas a nivel retrolingual, hipofaríngeo o supraglótico, mediante amigdalectomía lingual, reducción de base de lengua y/o supraglotoplastía. En pacientes seleccionados, esta técnica ha mostrado iguales o mejores resultados funcionales que las técnicas clásicas, junto con una baja tasa de complicaciones posoperatorias, sin embargo, implica un mayor costo asociado. A pesar de esto último, es una técnica válida y prometedora en el campo del SAHOS.


Abstract Obstructive sleep apnea and hypopnea syndrome (OSAHS) is a common condition that is associated with many negative consequences. First line treatment is usually medical management, but surgery is indicated for selected patients with retrolingual, hipopharyngeal or supraglottic obstruction. Transoral robotic surgery has been developed in the last years for the surgical treatment, through tongue base reduction and/or supraglottoplasty. In selected patients, this technique has shown equal or better functional outcomes than conventional techniques, along with a low rate of postoperative complications, however it implies a higher associated cost. Despite the latter, it is a valid and promising technique in the field of OSAHS.

ABSTRACT

Resumen: La práctica de cirugía robótica es un proceso complejo que implica desarrollo y tecnología no sólo en el campo de la cirugía, sino también en el campo de la anestesiología. Implica un proceso multifactorial, ya que ha generado un cambio drástico multidisciplinario basado en tecnología de punta que pretende ofrecer mejores condiciones durante el manejo perioperatorio en cirugía robótica. La anestesia total intravenosa cumple objetivos específicos en relación a la posición del paciente, relajación cerebral, neuroprotección, hemodinamia, pérdida y recuperación de la conciencia, parálisis neuromuscular, parámetros ventilatorios, etc. Ofrece seguridad y calidad al paciente durante el procedimiento con una mínima interferencia con el monitoreo electrofisiológico y permite modular la profundidad anestésica desde una neurosedación hasta una anestesia general, de acuerdo a las diferentes etapas de la cirugía. Un factor atribuible a la anestesia moderna para el éxito de la cirugía robótica es usar diferentes agentes anestésicos que promuevan inducción, mantenimiento y emersión anestésica más rápida y suave, a fin de reducir el tiempo de recuperación del estado de conciencia, funciones básicas y psicomotoras como la anestesia general multimodal.


Abstract: The practice of robotic surgery is a complex process, involving development and technology; not only in the surgery field but also in the anesthesiology field. It implies a multifactorial process since it has generated a drastic multidisciplinary change based on state-of-the-art technology; which aims to offer better conditions during perioperative management in robotic surgery. Intravenous Total Anesthesia accomplishes specific objectives in relation to patient position, brain relaxation, neuroprotection, hemodynamics, loss and recovery of consciousness, neuromuscular paralysis, ventilatory parameters, providing safety and quality during the procedure; with minimal intervention during electrophysiological monitoring and enabling anesthetic depth to be modulated from neurosedation to general anesthesia, according to the different stages of the surgery. A factor attributable to modern anesthesia for robotic surgery success is to employ different anesthetic agents promoting induction, maintenance of general anesthesia, smother and faster anesthetic emersion, for the purpose of reducing recovery time of the state of consciousness), basic and psychomotor functions; as is the general multimodal anesthesia.
